Maine Secretary of State Bureau of Motor Vehicles - BMV Forms and Applications · Maine state form

Birth Affidavit

This affidavit helps eligible driver’s license or identification card applicants establish U.S. citizenship when an acceptable birth document cannot be obtained.

What this form is
The applicant completes Section A, and a person with knowledge of the applicant’s birth completes Section B. The instructions say two separate affidavits from two different people are required. Applicants must obtain an official letter confirming that no birth record exists, or an official statement that a foreign birth record is unavailable, before using the form.
Who needs it
A U.S. citizen applying for a Maine driver’s license or identification card who cannot obtain an acceptable birth document may use this affidavit.
Why it needs a notary
The affidavit is subscribed and sworn under pains and penalties of perjury, so the signature must be notarized. This can be done online with a commissioned online notary.
How to submit it
Complete Section A, obtain two affidavits from different people, and attach the required official letter. Return to a Bureau of Motor Vehicles office with both affidavits to apply.
